1. Introduction

This Privacy Policy provides information on how your personal information, which includes your sensitive information and your health information, is collected and used by W Life Australia Pty Limited (ACN 695 776 160) trading as W Life (ABN 22 695 776 160), and the circumstances in which we may share it with third parties.

This Privacy Policy applies to personal information provided to us or otherwise collected by us online or offline, including through our website at wlife.co, our patient dashboard, associated applications, communications and other services we make available from time to time.

This Privacy Policy applies to the W Life programme as delivered in Australia by W Life Australia Pty Limited. Any programme delivered in another jurisdiction, or by another entity, is governed by the arrangements applicable to that jurisdiction and entity and is outside the scope of this policy.

We are bound by the Australian Privacy Principles contained in the Privacy Act 1988 (Cth).

2. Why and how your consent is necessary

When you register for the W Life programme, you provide consent for us, including our employees, agents, contractors and other representatives, to access and use your personal information so that the treating practitioner can provide you with the best possible healthcare. Only persons who need to see your personal information will have access to it. If we need to use your information for any other purpose, we will seek additional consent from you to do so.

Because the W Life programme involves genetic testing and testing for notifiable conditions, we seek specific, separate consent for those tests rather than relying on your general consent to the programme. You may decline any of them without affecting the rest of your assessment.

3. Why we collect, use, hold and share your personal information

We collect your personal information to facilitate the provision of the W Life assessment and associated healthcare services to you, and to assist in the management of your health. We also use your personal information for directly related business activities, such as payments, practice audits, and business processes including staff training.

5. Dealing with us anonymously

You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so, or unless we are required or authorised by law to deal only with identified individuals.

Because the W Life programme involves pathology, imaging and genetic testing performed by accredited third-party laboratories, and the accurate matching of results to the correct individual, it is generally not practicable for us to deliver the programme on an anonymous basis.

Please also be aware that Medicare rebates, where they apply to clinically indicated testing outside the programme fee, are only available where Medicare details are provided.
